When staff are supervising children, they shall always ensure their care, protection, and guidance. On March 19, 2026, Staff #1 was gathering the children to go outside. Staff #1 lined the children up and proceeded to do name to face. Before leaving the classroom, Staff #1 became distracted by a child who requires assistance during transitions. When the staff arrived to the gym before heading to the playground, name to face was not done. Child #1, (4-year-old), ran back to the classroom to get their water bottle. Staff #1 did not notice Child #1 leaving the group. The group arrived outside at approximately 4:05 and child #1 was found in the classroom by Staff #4 at approximately 4:20pm. Child #1 was not harmed or seemed distraught.
