Evidence: 1. According to an incident report dated 05/10/2023, Child #1 was climbing on the raised flower box and fell. Child #1's arms and legs were cleaned off by Staff #1 and checked for broken skin. Staff #1 did not observe any marks or broken skin and Child #1 returned to play. 2. At approximately 2:30pm, Staff #1 stated that she observed a mark on the inner right thigh of Child #1. Staff #1 stated that they assumed that the mark happened at home. 3. At approximately 4:45pm, the parents of Child #1 noticed a large mark on the inner right thigh of Child #1. 4. The wooden flower bed box located on the far left of the playground had splintering and deteriorating wood on two of the outer corners of the flower bed box. The deteriorating wood of the left corner of the box had an exposed metal bolt head that had sharp edges.
