On Friday, April 18, 2025, Child #1 (3 years-old) was left unsupervised for approximately 7 minutes within their classroom. Staff #1 did not notify Staff #2 (who was standing by the propped open door to the outside) that Child #1 was still in the room as they (Staff #1) exited.

Evidence - 1. On the date of inspection (01/26/2024), three bottles for Child #1 were dated - "11/27, 11/28, and 11/29" and not the current date. 2. This observation was confirmed by center staff on the date of inspection.

Evidence - 1. On the date of inspection (09/01/23), within the Infant II room, while bottles were labeled with each individual child's name, they did not include the current date. 2. Two of the ten bottles observed were labeled correctly with both the name and date.

Evidence - 1. The Center Director stated (during an interview conducted on the date of inspection) that on 06/14/2023, at approximately 11:25 am, within the preschool pathways 1 room, Child #1 (2 years of age) was left unsupervised for approximately 5 - 10 minutes. 2. This incident was a result of Staff #1 performing an incorrect name-to-face count of the 16 children leaving their classroom to go outside. 3. Child #1 was found by another staff in the classroom sitting and unharmed.

Evidence - 1. On the date of inspection (07/28/2022), within the bathroom located in the Older Toddler room, the upper lid to the left toilet was missing. 2. The Center Director stated that it broke 2 weeks ago.

Evidence - 1. On the date of inspection (09/20/2021), an Out-of-State Sex Offender Registry check (SOR) from the State of Florida was not available for review for Staff #3. 2. The Center Director stated (during an interview conducted on the date of inspection) that a SOR request for Staff #3 was sent out 09/07/2021. 3. Staff #3 was in attendance on the date of inspection. The start date of Staff #3 was listed as 09/07/2021.
