22VAC40-185-340-F · Based on staff interviews, the center failed to ensure that all children under 10 years of age were within actual sight and sound supervision of staff.
Evidence: 1.On 7/12/2021 at approximately 5:00 pm, Child #1 (2-year-old) was left behind in the Two's classroom while Staff #1 transitioned a group of seven 2-year-old children to the playground. 2. Staff #1 stated that during the transition from the classroom to the playground, Staff #1 lined up the 7 children to take them outside. She stated she did the face to name and thought Child #1 (2-year-old) was with her. She was unaware that Child #1 had been left alone in the classroom until Staff #2 brought the child to the playground and told her she left Child #1. Staff #1 stated they had been on the playground approximately 5 minutes. 3. Staff #2 found Child #1 inside the Two's classroom alone at approximately 5:05pm, reported the incident to Staff #4 and took the child to the playground to join their class. Staff #2 stated she told Staff #1 "You forgot her in the classroom." 4. Staff #3 viewed the camera footage and said the child was left alone from approx. 5:00-5:05pm.
