On 3/2/2026, Staff #2 observed Staff #1 move two-year old Child #1's hand that was soiled with feces towards the child's face, getting feces on the child's face and then asking the child, "do you like that." However, Staff #1 did not report the matter immediately to the department's toll-free child abuse and neglect hotline or to the person in charge of the child day program.

    When staff are supervising children, they shall always ensure their care, protection and guidance. On 6/3/2025, Staff #1 and Staff #2 staff were performing a face-to-name check while transitioning a group of children from a classroom on the third floor to another classroom on the second floor, when Child #1 ran ahead of the group and went down the stairs to the classroom on the second floor. The staff did not go after Child #1 until the group had finished the face-to-name check at the top of the stairs. Neither staff would have been able to offer assistance to Child #1 during this time away from the group.
