Evidence: 1. Child #1 (2 years old) from the Butterfly class was out of sight and sound supervision of staff for approximately 2-3 minutes during pick up . 2. Child was found unsupervised and alone in the (Busy Bee) classroom with door closed.

8VAC20-780-400-A · Based on interviews, staff failed to interact with one another to provide needed help, comfort and support.
Evidence: 1.On the day of the incident 10/26/2023 at approximately 9:15am, there were 3 staff members (Staff #1, Staff #2 and Staff #3) in the Busy Bees classroom, and 10 children (2 years old) in attendance. 2.Staff #1 and Staff #2 engaged in an argument and began to yell at each other. Staff #3 was in the classroom and stood between the two teachers stating: "This is not the place to do this". The argument continued to escalate and the yelling continued for approximately 3 minutes. 3.Staff #4 heard the yelling from the hallway, they walked into the classroom and they pulled Staff #1 out of the classroom. Staff #4 stated: "I pulled Staff #1 out of the classroom to separate them, because they were getting louder and louder and their body language indicated that the were going to get into a physical fight. They were in each other's face." 4.Staff #4 stated: "At some point Staff #1 got away from me and entered the classroom to remove their purse, but once they entered the classroom they continued to yell at each other." 5.Staff #5 was called to assist with the incident. Staff #4 and Staff #5 proceeded to pull Staff #1 out of the classroom and asked Staff #3 to lock the classroom door. 6.When questioned about the children during the incident, Staff #3 stated: "Some of the children were aware of the incident. One child got upset, he was whining a little bit."

Evidence: 1. On 10/16/2023, at approximately 9:20am, Child #1 (15 months old) was placed in a crib near the front desk with Staff #1, to wait for their parents to pick them up. 2. On the day of the incident, Adult #1 reported that as they were leaving the facility, they observed Child #1 in a crib near the front desk alone and unsupervised. 3. Staff #2 and Staff #3 stated that on the day of the incident they observed Child #1 in a crib by the front desk, alone and unsupervised. Staff #2 reported that Staff #1 stepped away from the desk and walked into a classroom, leaving Child #1 unsupervised for approximately 2 minutes.

8VAC20-780-500-B · Based on interviews, center staff did not clean and change a child's diaper immediately when it became wet.
Evidence: 1. On 8/10/2023, Child #1 (2 years old) was not cleaned and changed immediately after their diaper became wet. 2. Staff #2 stated that Staff #1 checked diapers in the morning, but Child #1's diaper was dry. The diaper report sent to the parents through the center's app documented that Child #1 was checked at 9:44am and at 12:44pm, both times it was documented that the child had a dry diaper. 3. During the afternoon, Staff #1 stated that they didn't change/check diapers after naptime because they were taking the children to the playground. Staff #1 stated that when they returned from the playground, they began to check/change diapers, but they did not get to change Child #1's diaper because a parent arrived to pick up their child and they were sidetracked. 4. Once Staff #1 left the center at approximately 4:45pm, the child's diaper was not checked again which resulted in a completely saturated diaper. Child #1 did not have a diaper change from the time the child was dropped off at approximately 8:52am until approximately 5:38pm when the child was picked up from the center.
