The center must ensure that the center?s activities are maintained in compliance with its own policies as required by the standards. The center?s supervision policy states that ?Staff should be aware of how many children are in their care and where all children are, at all times. The policy also states, ?Supervision of children (indoor and outdoor) - Know the names and the number of children in their care at all times, use transition tracking to account for the children in their care, conduct and record transitions with face to name roll call, conduct a head count whenever a transition through a door or gate occurs, conduct periodic head counts throughout the day, in addition to those required during transitions, log in and out whenever they join or leave a group. On 10/23/24 at approximately 6:18pm, three-year old Child # 1 was left alone and unsupervised on the playground for approximately two minutes. Staff # 1 and Staff # 2 were responsible for supervising a group of children on the playground and gathered the children to come inside at 6:18 pm. Both staff stated that they did not count the children and did not complete a face to name count, as per the center?s supervision policy.

Staff # 1 and Staff # 2's records showed that they resided outside of VA in the last 5 years. Staff # 1 has been employed with the center for over three months. The out of state central registry search for Staff # 1 was submitted two days after the first day of employment, the results for which have not been obtained to this date; and the center did not complete any follow up with the agency. Staff # 1 was observed working alone with children in the Preschool B classroom. Staff # 2 has been employed for 8 months. The out of state central registry search for Staff # 2 was submitted but the results have not been obtained to this date. The center did not conduct a follow up with the agency.

The documentation in Staff # 1 showed that she was not lead qualified. 4. Staff # 4's record had documentation of college education from an international institution that had not been translated. Program lead qualifications for Staff # 4 could not be determined.
