The provider did not ensure that all individuals working with children are qualified caregivers. On May 15, 2026 Household Member #1 was utilized as an assistant caregiver in in the family day home. Household Member #1 is not an identified assistant and does not have a record that contains all of the requirements of a family day home assistant caregiver. The provider was out of compliance on for approximately 3 hours.
