Mouse droppings were observed in classroom #1 around the base of furniture and on the carpet accessible to children, aged 12-16 months. Mouse droppings were observed in classroom #2 around the perimeter of the room accessible to children, aged 16-24 months. Mouse droppings were observed in classroom #3, which was occupied by children aged 2-3 years old, underneath a stack of cots. Mouse droppings were observed in classroom #4 on top of cubbies near the microwave, under a sink, and in the closet, accessible to children ages 3-4 years old. Mouse droppings were observed in classroom #5 on toy shelves, under the cubbies, and in a corner ,accessible to children aged 3-4 years old. Mouse droppings were observed in classroom #6 in the closet and in the reading area , accessible to children aged 4-5 years old. When staff are supervising children, they shall always ensure their care and protection. Based on the information available, the cause of injury could not be determined; however, Child #1, age 4 months, sustained a fractured leg while in the care of center staff. Witness A confirmed that Child #1 was calm with no visible signs of injury on the morning of 3/31/26. Video evidence provided by the program indicates that Child #1 later departed the center in apparent pain prior to seeking medical evaluation. Discoloration and swelling of Child #1's leg and foot was observed and documented by staff prior to contacting the parent at 2:00 pm. Witness A stated that Child #1 was observed "in distress, with redness and swelling in their foot," which is when they became concerned and reported the injury to school administration. Following medical assessment, Child #1 was diagnosed with a fractured leg.

The facility did not follow their "Threshold Counts and Name-to-Face Checks Policy" on 11/21/2025 when transitioning the Preschool 1 children to Preschool 2 at the end of the day. The Policy states: "As children leave the classroom with their teacher(s), threshold counts and name-to-face checks are conducted to ensure that all children are accounted for with the group. Threshold Count: This is the practice of a teacher counting the number of children in line and matching that number to the current number in attendance. Threshold counts occur at every doorway, stairwell, or threshold along the way to your final destination. Name-to-Face Checks: Once the class has reached the final destination, a name-to-face check occurs. The teacher matches the name on the attendance record with the face of the child as he/she crosses the final threshold. A system for the name-to-face checks is recorded or evident via tablet transitions, paper attendance, or other system in place. A verbal confirmation of the name-to-face checks occurs (i.e. if transferring children in a tablet, say names aloud as well)." On 11/21/2025, Child 1 was left alone inside the bathroom located in the Preschool 1 classroom from approximately 5:20-5:25pm. There was no verbal confirmation of the name-to-face check occurring at the time of the incident, nor was there accurate documentation reflecting where Child 1 was when the Preschool 1 children transitioned to the Preschool 2 classroom at the end of the day. Staff did not follow the center's own policies and procedures.
