8VAC20-780-340-A · Based on staff interviews, the center failed to ensure the care, protection, and guidance of one child, when supervising children.
Evidence: 1. On 3/23/2023, a 3 year old child was unsupervised for an undetermined length of time. Attendance records indicate a Face to Name check was completed at 1:30pm during which Child #1 was present. At an unknown time following the face to name check, the whereabouts of Child #1 were unknown. Child #1 arrived back in the classroom with Staff #3 at approximately 1:45pm. 2. On 3/23/2023 during naptime, both Staff #1 and 2 report Child #1 was located on his cot, but awake and moving around. Sometime after 1:30pm, staff noticed Child #1 was no longer on his cot; however neither staff member recall seeing Child #1 stand up or leave his cot. Both staff report the children are able to freely go to the restroom, which is located in the classroom, during naptime. Upon discovering Child #1's absence, staff searched the classroom, the bathroom and the adjoining empty classroom. Staff #3, who was returning from a break continued to search, entering the hallway outside of the classrooms. A school employee where the program is located was seated in the hallway and indicated that the ?little boy went that way? and pointed towards the main office. Staff #3 walked to the front main office of the school where she found Child #1 with a school employee. Staff #3 returned Child #1 to the classroom at 1:45pm.
