The center has a supervision policy that states ?each staff will account for the children in their care on their checklist before an individual child or a group transitions to a specific location,? ?be clear about the role each staff is responsible for during transitions,? and ?two staff should be responsible for seeing that the transition flows smoothly. One should initiate the transition by leading the group into the new area. The other makes sure that all the children reach the new area and none are left behind.? On December 30, 2025, seven staff transitioned a group of children from the playground to the classroom. Child #1 (age: 5 years) was left on the playground, unsupervised. Multiple staff reported that the children were not accounted for on the accountability sheet before they transitioned to the classroom. Staff also stated that the role each staff was responsible for during the transition to the classroom was not clear. During the transition, no staff made sure that all of the children reached the new area and none were left behind.
