SYSTEMIC DEFICIENCY The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department. On Friday, September 26, 2025, video footage from Classroom #1 showed Staff #1, Staff #2, and Staff #3 not following the center's own policies and procedures that are required by the standards regarding procedures of action in case of injury and discipline policies including acceptable and unacceptable discipline measures. The center?s procedures for action in case of injury required teachers to use accident reports to record and communicate a child?s injury or an incident. The report included what the injured child was doing prior to getting hurt and how the teacher responded. Teachers addressed the situation with all families involved either by phone during the day or at pickup. The center?s discipline policy defined unacceptable discipline as: roughly handling or shaking a child, restricting movement through binding or tying, enclosure in a small confined space or any space that the child could not freely exit, withholding or forcing of food or rest, etc. Staff #1, #2, and #3 signed the center?s orientation form acknowledging the center's policies and procedures required by the standards.
