Based on records review and staff interview, the center did not follow its own policies and procedures that are required by the standards. Evidence: 1. The center?s ?Supervision of Children? policy states, ?Enrolled children must be supervised at all times. This means that staff members must never leave a child or children unattended for even a moment?. The policy also states that, ?Leaving a child in the classroom while others move to another room are all examples of lack of supervision?. 2. The center?s ?Child Supervision Records? (CSR) policy states that ?Name to face attendance (visually confirming children are present by matching the names on the CSR to the faces in the classroom) must be taken every 30 minutes and during any large class transitions (i.e., going outside to the playground). 3. On 11/2/23, at approximately 10:10 am, Staff # 1 and Staff # 2 left Child # 1 (1 year old) unattended in the classroom for approximately 2 minutes when they left the room with 7 other children to go to the playground. 4. Staff # 1 stated that at approximately 10:10 am, they were lining up to go outside for the fall festival and were running behind. Staff # 1 stated the children were not lined up, and she did not do a name to face count using the CSR. Staff # 1 thinks that Child # 1 was unattended for approximately two minutes from the time they went outside. 5. Staff # 2 stated the children were lined up to go outside, and Staff # 2 did not do a name to face count using the CSR. Staff # 2 stated that Child # 1 was unattended for approximately two minutes.

During this inspection, as CSRs were being reviewed, Staff # 2 stated that on 2/27/23 and 3/2/23 she arrived to work at 9:00am in the Preschool/Pre-K (3?4-year-olds). Her timesheets for both dates confirmed this information. However, her arrival time on the CSR for both dates had been altered to 8:00 am. Staff # 2 stated she had not made any changes to her time on these CSRs. 4. The CSR for the Preschool/Pre-k classroom dated 2/28/23 did not have any initials or documentation of verification to demonstrate that the center?s management had checked children?s time in/out as per the center?s attendance policy. 5. The CSR for the Discovery Preschool (2?s) classroom, dated 3/2/23 was missing the ?depart time? for Child # 1. 6. On 3/2/23, Staff # 3 did not document his times in and out of the classroom correctly on the CSR for the Preschool/Pre-K classroom. On one line of the ?Teacher/Staff Attendance? column, Staff # 3 had documented that he had left the classroom at 9:02 am. On another line his time-in was documented as 9:00 am and time-out as 5:05 pm.

Staff # 1 stated that while on the playground, Child # 1 (3-years old) jumped in a small puddle of water, left from the rain the day before. Staff # 1 noticed that Child # 1?s pants had gotten wet from the water up to his knees. 3. When they returned back to the classroom, it was time for Staff # 1 to leave on lunch break. Staff # 1 told Staff # 2 to change Child # 1, but did not specify that Child # 1?s clothing had gotten wet. At approximately 11:03 am, Staff # 1 left on break. 4. Staff # 2 stated that when Staff # 1 mentioned Child # 1 being wet, Staff # 2 thought that Staff # 1 was referring to Child # 1?s pull up, and did not notice that Child # 1?s pants were wet. Staff # 2 stated that they got busy with handwashing and fixing lunch, and did not get a chance to check Child # 1?s pull up. 5. After lunch, the children got on their cots for nap time. Staff # 2 stated that they were going to check on Child # 1 but by that time Staff # 1 returned from break. 6. Staff # 1 returned from break at approximately 12:03pm and saw that Child # 1 was asleep on his cot, covered by his blanket, and assumed that he had been changed. 7. Staff # 1 stated that at 2:35 pm Child # 1 woke up. When helping him up, Staff # 1 realized that Child # 1?s pants were still wet. Staff # 1 stated that ?Child # 1?s pants, socks and shoes were soaking wet?, and when he got up from his cot, his cot cover was also wet. 8. Child # 1 was in wet pants, socks and shoes from approximately 11:00 am to 2:35pm.

The toddler classroom's CSR dated 6/4/21, showed that Child # 2's time out was 5:30 pm. Staff # 2 's initials were documented on the first page of the CSR under "name to face checks". However, Staff # 2 did not sign into the Teacher/Staff Attendance box on the back of the CSR. 4. Additionally, Staff # 2 did not sign in to the Teacher/Staff Attendance box for the toddler CSR during the entire day, even though Staff # 2's initials were documented on page 1 from 7:29 am to 5:40 pm. 5.
