On April 29, 2026, Child #1 (16 months old) had been climbing up a slide and was picked up and redirected three times prior by Staff #2. When Child #1 began climbing the slide again, Staff #2 told them not to climb up the slide, but did not physically redirect. Child #1 fell, causing an injury to their mouth which resulted in breaking a tooth. Staff #1 and Staff #2 did not witness the fall, as documented in the child?s file.
