Broken pieces of plastic were found spread out on the grassy area under the play equipment. 3. A broken toy saw was found in the play equipment. 4. Debris and clutter was found in and around the climbing play equipment.

The provider stated that she cared for Child # 1 at the other home for most of the week, from 7:15 am to 6:00pm. The provider also stated that care provided on the main level of the home, primarily in the family room.
