Broken pieces of plastic were found spread out on the grassy area under the play equipment. 3. A broken toy saw was found in the play equipment. 4. Debris and clutter was found in and around the climbing play equipment.

Evidence: 1. During the week of 4/24/23, the family day home was closed and care for Child # 1 was provided at another residence. 2. The provider stated that due to some home repairs she had to go live at another family member's house close to the family day home. 3. The provider stated that she cared for Child # 1 at the other home for most of the week, from 7:15 am to 6:00pm. The provider also stated that care provided on the main level of the home, primarily in the family room.

Caregiver #2 stated that from approximately 9:30 am to 1:00pm (3.5 hours), Caregiver # 2 would have to leave, and Caregiver # 1 was alone with the children in care.
