Evidence: At the time of this incident, there were 22 children in care with three staff members. Staff 1 was conducting head counts to transition children to the playground, Staff 2 was on the playground, and Staff 3 was assisting with the classroom transition process. At approximately 10:54 Staff 1 is seen on video completing a name-to-face roll call. As each child's name is announced, the child is seen walking to the playground door then exiting the classroom. At approximately 10:56 Staff 1 appears to call the name of Child A, however the child did not proceed to the playground. Instead, Child A went to the sink in the corner of the classroom. At the same time, Staff 1 is seen completing the name-to-face roll call process, closing the classroom door, and proceeding outside with the children. At this time, the video shows Child A walking around the classroom alone and beyond sight and sound supervision of staff members. Roughly 2 minutes later, at approximately 10:58, Child A is viewed opening the playground door and exiting the classroom. Through staff interviews and written statements, it was determined that of the three staff present at the time, none were aware that Child A remained in the classroom beyond sight and sound supervision. They were not aware that the child was alone in the classroom until the child opened the playground door and proceeded outside.

Evidence: 1 - On 11/8/2022 there was one staff with 16 children in the Pre-K A classroom between approximately 8:35-8:45AM. 2 - On 11/9/2022 there was one staff with 18 children in the Pre-K A classroom between approximately 8:20-8:45AM.

Evidence: 1 - On 12/31/2021 Staff A received email notification that while in care on 12/30/2021, Child 1 (age 2) obtained injuries to the left arm including a scratch that broke the skin and a dark mark next to the scratch. The email included two pictures of the child's injuries showing the broken skin and the dark mark. The email also included statements noting that the child indicated the marks were the result of a physical interaction with Staff B. As a mandated reporter, Staff A was required to immediately report this information to Child Protective Services. 2 - During interviews with Staff A, it was determined that they failed to report the incident of suspected child abuse to Child Protective Services.

Staff #1 took Child #1 to a corner of the classroom located by the bathroom and Child #1 was in the corner area of the classroom for approximately 10 minutes, isolated from other children. 2. Staff #3 and #4 were in the classroom with staff who rough handled children and did not report the concerns to administration per policy.

The center?s safe sleep policy requires that staff use infants will always be placed on their backs to sleep, unless medial documentation is present in the file sating otherwise. 3. The center?s safe sleep policy requires that infants will be moved to their beds if they fall asleep in a swing or bouncy seat, or any other area besides an infant crib. 4. Child A (3-month-old) was observed asleep on a bobby pillow face down.
