Evidence: 1. The center has a bottle feeding verification policy that states, ?before giving a bottle, the caregiver feeding the bottle (the feeder caregiver) and another caregiver (the verifier) must verify that the correct bottle is being fed by following the 4 step process.? 2. Staff #1 (the verifier) stated that they grabbed a bottle for Child #1 and handed it to Staff #2 so that Staff #2 could feed Child #1. Staff #1 did not complete the 4 step bottle verification process before Staff #2 gave the bottle. 3. Staff #2 (the feeder caregiver) stated that Staff #1 asked them to feed Child #1. Staff #1 handed Staff #2 a bottle belonging to Child #1. Staff #2 fed the bottle to Child #2 after thinking that it was Child #1. Staff #2 did not complete the 4 step bottle verification process of verifying the correct bottle and the correct child, before giving the bottle. 4. Staff #1 and Staff #2 both stated that they were aware of the center?s policy on bottle feeding verification.

In the ?Pre-K A? classroom, there was chipped paint under and to the left of the window that looks outside, chipped and peeling paint under the window by the technology center, chipped paint to the left of the cubbies, chipped paint on the back wall beside the bulletin board, and chipped blue paint on the chair rail. 5. In the ?Toddlers B? classroom, there was chipped paint on the wall by the cots and chipped paint by the circle time carpet. 6. In the ?Pre-K? classroom, there was chipped paint on the door frame of the right bathroom and chipped paint on the wall by the circle time carpet.
