Evidence: 1. There are the following concerns on the outdoor playground of the center: a. The pink Little Tykes car has two protruding screws on the front of the car that pose as a snagging and injury hazard to the children in care; b. The large black tire that sits on the ground has accumulated water inside of the rim which poses concerns of stagnant water to attracts various bugs or animals to harm the children in care; c. The blue truck that children climb on has a missing steering wheel and there is a 1 inch screw that is exposed and loose. This poses as a possible snagging, choking, or injury hazard to the children in care; d. The plastic picnic table and bench set, which is the height and size of the children in care, is broken as it has loose parts to the table and very unstable. Should children attempt to sit on the bench, it will immediately fall and it poses as an injury hazard. 2. Staff #1, Staff #2, and Staff #3 confirmed that the playground concerns listed above are broken and are in need of repair or replacement.
