After being aware of a child who was able to open the playground gate with ease on more than one occasion, immediate action was not taken to better secure the gate. Video of an incident was observed where the child opened the gate and ran before being retrieved by the caregiver on 3/25/26. At the time of the investigation inspection on 4/2/26, it was observed that a correction to better secure the gate had not yet occurred, which showed continued risk.

The child had several abrasions across the face, a bruise and later began to display signs of a possible concussion prior to being picked up by the parent. The caregiver also did not ensure that the director, who was still present at the time, was properly notified. The director was not made aware of the incident until after the child was picked up by the parent.
