Based on video footage obtained, one staff member was observed grabbing an infant in an aggressive manner when she moved the infant from one area to the carpeted area and quickly turned the infant's body to the mirrored shelf. The staff grabbed the infant?s left arm in attempt to prevent the child from maneuvering from placement.

Two staff observed in the kitchen area was not aware of the ages of the youngest child in care. The youngest child in care was introduced as 6 months when the proper age is 14 months of age. The caregiver in the Infant 3 classroom was not aware of the age of the youngest child in care. Operation staff was provided proper ages of children/infants in which they were responsible for.
