  A caregiver was present with an inactivate background check. This was corrected at inspection when the caregiver left the operation.

Information obtained through the investigation show that children were not appropriately supervised when one child went under a table where they received an injury to their back. There is information to support that children were also kicking this child. Information obtained through the investigation shows that children are allowed to go to the supply closet without supervision.
