Based on the information obtained during the investigation a caregiver left a child outside unsupervised for approximately 2 minutes. Note: The caregiver in charged was terminated. Two other caregivers were wrote up.

Based on the preponderance of information obtained before the investigation the owner admitted to the incident not being reported within 2 days of the injury and medical attention. Note: This was already in compliance by the time of the investigation when the owner reported the incident after speaking with the licensing rep about the situation.

Based on the information obtained before the investigation started the owner admitted that no incident report had been filled out for the incident. Note: This was corrected before the violation was issued the owner filled out an incident report when it was advised that it needed to be done and turned it into licensing.
