Based on the information gathered, a caregiver entered the facility with a vape pen in her shirt pocket. The pen fell out of her shirt resulting in a toddler picking it up and putting it in the mouth. It was retrieved quickly from the caregiver. NOTE: The caregiver with the vape pen was terminated.

Based on the video, it was determined that positive methods of discipline were not used when a caregiver yanked a child several times by one arm as the child was upset after morning drop off and was trying to walk away as the caregiver was talking to her. The caregiver was seen tossing the child inside of a tent area that had pillows located in the room. Note: The operation immediately terminated the caregiver.

A 3-year-old was bumped by another child while one was going down and the other one was coming up the stairs of the playscape resulting in the 3-year-old falling through an opening and landing wrong on her arm fracturing it in several places. The caregiver did not see the incident.
