Based upon interviews, staff did not do a name to face after transitioning children back into the classroom from a bathroom break. This resulted in a child being left unsupervised in the hallway near the classroom for approximately 1-2 minutes until the child was found by the director.

The operation did not notify Licensing about two positive COVID cases within the 2 days allowed. This was corrected when the director returned to the operation and completed the self-report email to the shared mailbox that was received on 02/15/22.
