Not all probation conditions were met regarding conditions pertaning to caregiver observations. There were some blank areas not filled on observation forms (comments, suggestions). Two observations of two different employes had same date of 8/29/23 and same time frame T1B 8:18 am - 8:50 am and T2B 8:20 - 8:50 AM time. One observation did not have a caregiver name. Some of the observations have the name of a director assistant and not the director's name, conducting the observations. There have not been any monthly conferences conducted with observed caregivers.
