Two infants were observed asleep in restrictive devices; one was asleep in a high chair and another infant was asleep while placed in an infant seat. After it was brought to the caregivers attention that young infants that are non-mobile must nap in a crib, the caregivers removed the infants and placed each infant in a face up position in a crib.

The caregiver was not in close proximity or visual aware of a child's on-going activity during outdoor play which resulted in the child's head entrapped between 2 parallel bars that encloses a drain on the pre-k playground.
