  During inspection eleven infants whose ages ranged from 3 months to 10 months old were present in the "Infant A" classroom. These infants were cared for by four caregivers. The maximum group ratio for this age group is 10 children for two or more caregivers. The oldest infant was transitioned into the "Infant B" classroom by the operation's director to comply with minimum standard requirements during inspection.

Based on DFPS Investigation a child escaped the operation and was left unsupervised.
