There was a toilet brush in an unlocked cabinet under the children's sink, a caregiver's backpack on the floor in the infant room within reach of children, a cracked toy and loose fence boards on the playground, and a loose outlet plate in the toddler room. During the inspection, the caregiver put the backpack up out of the reach of children and the director removed the toilet brush from under the sink, removed the broken toy from the playground and secured the loose outlet plate with tape.

During the inspection, piles of gathered leaves, grass, and various sized rocks were observed on the large playground. The toddler playground had an outside opening that would allow rodents to enter and drop pest droppings that can cause health problems and illness.
