A supervision incident occurred as a result of a caregiver using her personal cell phone while on the job. The caregiver was terminated by the director.

A wooden barrier at the base of a chainlink fence on the playground had a screw exposed which was accessible. A portion of the chainlink at the top of the fence was detached. Photos were taken. There was a small water leak in one of the classrooms which was closed. The leak was being addressed by a certified plumber at the time of inspection. There was no visible mold observed.
