Based on information gathered in the investigation it was determined the caregiver of a child left unattended in the classroom did not notice the child was missing when they got to the playground and during their time outside. They did not realize the child was missing from the group until they were alerted by someone picking up a child. This substantiates that the caregiver did not know which children (or how many) they were responsible for. Information gathered indicated transition protocols were not followed.
