Based on the DFPS investigation, it was found that the operation staff did not communicate with the caregiver regarding child information and information about which children were potty trained.

Based on the DFPS investigation and after reviewing the camera footage, it was found that the caregiver was not supervising the children. Children were observed climbing on tables, a child pulled a chair that was holding the restroom door open, and the child inside was left in the restroom and the caregiver was not interacting with the children.
