When reviewing the personnel records, it was found that one record was missing the required notarized Affidavit for Applicants. The director stated that she feels that it was mistakenly put in another file as everything else was completed including the approved background check form, but indicated that if they could not find it, they would ask the caregiver to complete a new form and have it notarized.
