A caregiver was observed coming in the operation with the class for gym time and did not complete the transition name to face transition form as required by the operation. The caregiver did not use good judgment when this name to face transition was not completed which ensures all of the children are with the class as required and supervision is met. There were 3 opportunities for this to be completed. This was addressed by the operation during inspection.
