During the unannounced inspection, diaper changing procedures were observed in two classrooms. In both classrooms, the caregiver was observed to allow the child to return play activities without washing their hands. This creates unsanitary conditions and places children at risk of illness.

One classroom was identified where the nap mats were not labeled with the child's name and no other identification process was used. Sheets were placed on the nap mats and were not changed during the week unless soiled. Children were allowed to sleep on any mat each day, and therefore, were allowed to sleep on sheets that had been previously used by other children.
