Two slide equipments in the playground areas were observed without surfacing in their use zones. The height of the highest designated play surface on the equipments were over 20 inches tall. NOTE: This was corrected during the inspection by Director of operation. Both slide equipments were removed from the operation.
It was determined that a caregiver duct tapped children's shoes, to prevent them from taking off their shoes.
