Based on the information gathered during the DFPS Investigation, a child was left unsupervised for approximately 20 minutes. Caregiver was unaware of child's whereabouts. Child was found in the roadway by a passerby and brought back to the operation, unharmed.

Based on the information gathered during the DFPS investigation, it was concluded the director allowed one caregiver to be responsible for 8 children which included 3 children with special needs, and 1 infant. A child managed to exit the operation without caregiver's knowledge.

Based on the preponderance of evidence obtained it is concluded that the caregiver in the operation did not use a positive method of guidance when directing themselves to a child in the operation.
