  A play yard was observed to have a loose fitted sheet placed on the mattress of the play yard.

This standard is tasked as part of an investigation and is found to be deficient. A caregiver removed a child's sweatshirt as a form of discipline. Removing a child's sweatshirt as a form of discipline does not clearly connect the consequence to the behavior of the child's level of understanding.
