One 20 oz can of pineapple chunks and one 14.5 oz can of whole corn were served to feed a total of 20 children. No eating utensils were provided, so children were eating with their hands. There was not enough food provided for seconds.

A large tree that has rotted and is dead, is adjacent to the play yard and had large chunks of bark falling of and possibly presents a risk of the tree trunk falling and causing injury to children inside and outside of the building. There was paneling observed coming away from the wall and showing signs of splintering and this appears to have been caused by water leaks. A window right below this paneling has dark substance that appears to be mold.
