The operation could not provide proof of monthly fire extinguisher checks. I spoke with the director about doing the checks each month after the fire drill is completed. The fire extinguishers were checked during the inspection.

During an investigation it was found that a caregiver slipped off their shoe while disciplining a child and stated something like "you know I dont do that", and then slipped their shoe back on.

Observed a 17 month old infant in the same classroom as children who were 7 years old. There was more than an 18 month gap between the youngest and oldest child in the classroom.

On the small playground there was a tote with electrical cords and a tool where the children can reach, there was also a piece of wood laying against the fence that could fall on children. There were pieces of wood stacked on the infant play area and the stairs were blocked with wooden raised flower boxes and stools that could fall on children. There was a greed rug hug over the railing of the stairs. The director was able to remove these items from the playground.
