A frame for a canopy on the toddler playground has two broken pipe pieces that are suspended and only attached to one end. One piece is suspended at a 45 degree angle pointing to the ground. The canopy frame requires immediate repair to prevent the broken pieces from coming completely detached and falling into the outdoor space. The piece that was at a 45 degree angle was repaired at inspection and the other piece will require a taller ladder to reach for repair.

One placed the child in time out and withheld snack time while the rest of the classroom had snack. There were a few incidents that required first aid and first aid was not given. A child in care tripped and injured his mouth and nose that resulted in bleeding. The child was brought it to be evaluated but after returning back on the playground the child's nose began bleeding again and the caregiver used his shirt to wipe the blood off his nose and mouth and then had him continue playing without requesting the first aid kit to additionally review his injuries. Additionally a second child had a bloody nose that was smeared across his face that was not attended to. Additionally, a third child pinched their finger under the bottom of the door as it was opened when entering the building from the playground and first aid was not provided.

The operation has areas that require more immediate attention regarding the maintenance of the building and grounds. Both playgrounds have an abundance of fire ants that have bitten several children in the areas that are frequently played on. Both playgrounds have an abundance of sticker burrs that are sticking the children's clothing and shoes. Then the sticker burrs are being tracked back into the classrooms and sticking on to the carpet. The interior of classrooms have dirty tiles, dirty carpets, and a fan has large layer of dust over the fan blades and the fan cover. The window sill has dirt, dust and dead insects.

There were several trash cans without lids that were accessible to children that had items that could attract pests and have. Additionally, a cord to a radio is expended down a tall cabinet to the used outlet. The cord is not secured, and the radio could be pulled down easily.
