When preparing to transition from the bathrooms, the caregiver-maintained eye contact with her daily attendance tracking sheet when writing down a check mark for each child in her responsibility. While marking her check marks, a child left the line and went back into the classroom. The caregiver didn't realize she didn't have this child until she finished marking the children in attendance. If the caregiver had been looking up between each check mark to do the name to face check correctly, the caregiver would have been providing a better level of supervision in addition to ensuring the name to face count was conducted correctly. The caregiver located in the classroom did notice the additional child immediately to provide adequate supervision. Additionally, when transitioning from the classroom to the playground a second time and this time with two caregivers, one initialed after the name to face count at transitions but it was not initialed by the second caregiver.
