It was determined through the course of the investigation that a caregiver demonstrated a lack of good judgment by holding the glass classroom door closed for approximately 21 seconds, preventing a two-year-old child from entering the classroom while the child attempted to open the door.

It was determined through the course of the investigation that a caregiver acted in an impatient, irritated manner when changing a 9 months old diaper by swatting at the infant in the face with the infants pants, aggressively slamming down the wipes container on the changing table, and using quick, rough, non-nurturing movements with the infant while changing their diaper.

Three of the 2 year old classrooms were observed to have cleaning supplies and sharp scissors in cabinets located under the sinks that were not locked and in reach of children. A potty chair in another 2 year old room had dired urine in the potty. This was all corrected at inspection by moving all the items out of the reach of children. Placing locks on the cabinets and sanitizing the potty chair.
