This occurred in the preschool 2 classroom. The children in this classroom are 2.5-3 year olds. The ratio at the time of the incident was 2:15 This agency usesa program called Tadpoles (on Ipads) for their name to face recognition As the children were coming in from the playground one educator was just inside the building with the children who had been marked off on the name to face recognition and the other educator was standing in the doorway calling the children inside and giving them a sticker as she marked them off on the name to face recognition on the Ipad. The child who was left alone came to the educator and received his sticker and was marked on the name to face document as having returned to the building but then instead of coming inside the building he darted back onto the playground and went inside the play structure where he could not be seen. The class proceeds to their classroom and upon conducting another name to face as the children entered the classroom, it was noticed that the child was not there. Another class was coming through the hall and another child sees the boy on the playground alone and says something to her teacher, this teacher notices the child and opens the door and calls for the child to come inside. As the child was coming inside, his teacher had arrived to get him because she has noticed he was not with the rest of the class. The child was left alone for just under 2 minutes. Video was viewed Statements from each educator were recieved A copy of the icident report was recieved A printed copy of the name to face document was received Valid complaint/incident of child being left on playground. The following violations were recieved: 1240-04-01-.11 SUPERVISION. (a) For children six (6) weeks of age through nine (9) years of age, the adult must be able to hear the child at all times, must be able to see the child with a quick glance, and must be able to physically respond immediately.
