Summary of Complaint Investigation Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and the investigation of a complaint. Technical Assistance Provided: 1. LC and Director discussed keeping numbers low by halting enrollment, until August, in a couple of classrooms in order to ensure that there are enough educators on site at all times which determines that they are staying in compliance all throughout the day. 2. LC and Director discussed getting her authorized for admin access on the Portal. No violations were given at this visit.

Observation: Licensing Consultant observed through staff interviews that an educator in the 2 ½-3 ½ year old classroom used inappropriate behavioral interventions when physically restraining a child on July 12, 2021. 1240-04-01-.15 Program, Language, and Literacy Development Corrective Action: We are following the safety plan put into place. All staff will receive training from CCR&R on behavior management and challenging behaviors.
