Observation: During an unannounced transportation visit, Program Evaluator Jones observed that the driver for the bus took out the roll sheet, wrote down the time, and checked all children off the bus prior to all children exiting the bus and put the roll sheet away before observing each child get off the bus and recording it. 1240-04-01-.17 TRANSPORTATION. Observation: During an unannounced transportation visit, Program Evaluator Jones observed that the bus only had the first 6 digits for the agency’s phone number listed on the vehicle and was missing the final 4 digits. 1240-04-01-.17 TRANSPORTATION. Corrective Action: The agency will get the last four digits of the phone number and place them on the bus.

Observation: During an unannounced transportation visit, Program evaluators Jones and Gipson observed that the drivers file for a new driver did not contain a clearance letter from the department stating she is ok to drive, nor a current CPR and First aid certification as the certification on file expired 2/2019, nor verification of transportation training in the last 6 months with the last training being from September of 2018. 1240-04-01-.17 TRANSPORTATION. Corrective Action: The agency will pay more attention to the files of drivers transferred from other centers to ensure all of the correct information is on file
