Summary of Complaint investigation and safety plan monitoring Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and Complaint investigation and safety plan monitoring. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. LC arrived at Cole Academy and immediately obtained ratios in each classroom. The Creeper classroom (18m-30m) had a ratio of 1:6 and then another arrived at the classroom became out of ratio 1:7. The educator did call to notify the assistant director and was told to wait a minute until she could find someone. LC arrived and obtained ratios in each classroom. LC observed the Toddler classroom(18m-30m) being out of ratio 1:9. LC arrived at Cole Academy and immediately obtained ratios in each classroom. The Pre-K B (4y-5y) was out of ratio 1:18 and the educator stated that she had been out of ratio since 9:06 am.

In the afternoon around 5:30 pm the agency combines children ages 13 months to 5 years old together A Child was left unattended for approximately 8.5 minutes Management is responsible for hiring qualified staff, and then supervising their performance to ensure they are capable of their job responsibilities. The management's failure to do this has led to a staff member using inappropriate language and behavior with the children. The parent works at the agency and was aware that the child was missing but no incident report was given or in the child's record
