See Below for Detailed Information Observation: On the afternoon of 05/03/2021, an educator failed to complete a required roll call before leaving the classroom to take children to the playground, resulting in a child being placed at risk of harm, when he was unintentionally left unattended in the classroom. 1240-04-01-.08 SUPERVISION. (3) Playground Supervision. (b) Childcare agencies shall develop and follow a written playground supervision plan that Includes: 1. Arrival and departure procedures. 2. Playground design and placement of equipment. 3. Individual staff duties to ensure age-appropriate supervision can be given to each child. 4. Individual children’s needs including high risk behaviors. 5. Emergency procedures, including communication with other staff; and 6. Roll call before leaving the classroom and upon arrival at the playground, and prior to leaving the playground, and upon arrival back in the classroom. Corrective Action: All staff will participate in re-training on supervision with an emphasis on transitions, and roll call requirements. This training provided by LC Sowell. Observation: On 05/03/2021, a child, age three years, was put at immediate risk of harm when left unattended in a classroom for approximately 4 minutes. 1240-04-01-.11 SUPERVISION. Corrective Action: All staff participated in training in supervision with an emphasis on transitions and the requirement of name to face roll calls.

Observation: An individual was working without a cleared background check on 4/5/22. 1240-04-01-.07 CRIMINAL BACKGROUND CHECK ANDSTATE REGISTRY/RECORDS REVIEW PROCEDURES. Corrective Action: The agency director thought she had received a cleared background check letter. When the error was discovered, she sent the individual home immediately. She will be more diligent in the future with her record keeping and review.

Observation: Licensing verified that an employee called one particular toddler an inappropriate nickname. Regardless of the intent, this is viewed as inappropriate and potentially shaming and humiliating. 1240-04-01-.15 PROGRAM, LANGUAGE, AND LITERACY DEVELOPMENT. Corrective Action: The Director planned to meet with staff on 05/21/2019 to address this violation and review the rule. In addition DHS Licensing and CCR&R will provide additional training for all staff.
