CI 1.1: Administration of medication

Missing Evidence: • Medication and preventative products authorized by proper parental documentation, received, and administered by designated staff. All unused medications returned to parent. (Nighttime Gripe Water found with a syringe in the bottle in the Infant Classroom. Educator stated that the mother gives the child the medication in the mornings. The child's mother is an educator in the toddler classroom at the agency.)

CI 1.6: Prevention and control of infectious diseases (including immunization)

Missing Evidence: • Prevention and control, including the spread of infectious diseases. (In the School-Age Classroom, the tables were not cleaned prior to lunch. Educator did not wear gloves during serving/assisting with lunch. Children not required to washing hands after lunch.)

CI 1.7: Building and physical premises safety

Missing Evidence: • Items labeled "keep out of the reach of children" are locked when not in use with the exception of hand soap. Bleach water or disinfectant/soap water must be inaccessible when not in use. (Infant Classroom items inaccessible but not locked - disinfectant spray, baby wipes, lysol wipes. Toddler Classroom has Lysol spray inaccessible,but not locked. Threes Classroom has cleaning items on an inaccessible shelf, but not locked.)

CI 1.10:Supervision

Missing Evidence: • Playground supervision must be maintained at all times. The playground supervision plan must be followed. • Children must never be left unsupervised. (The School-Age Classroom was on the playground. A child needed to use the restroom and was instructed to go inside without supervision to use the restroom.)

E 1.1: Food sanitation procedures followed.

Missing Evidence: • High chairs and tables, on which food is prepared and served, shall be washed with soap and water and sanitized directly prior to and after snacks and meals. (The School-Age Classroom was observed during their lunch. The tables were not washed with soap and water and sanitized before the meal.)

E 1.9: Mealtime Supervision

Missing Evidence: • Food shall not be given to a child until the supervising educator is able to provide focused attention to the child. (Educator in the School-Age Classroom was in the restroom washing her hands as children began eating their lunch.)

E 1.10 Playground Supervision

Missing Evidence: • Roll call before leaving classroom and upon arrival at playground and prior to leaving playground and upon arrival in classroom. • Individual staff duties developed and followed to ensure age-appropriate supervision can be given to each child at all times. (Children in theschool-age classroom are permitted to go inside to use the bathroom without proper supervision.)

E1.5: Activities with planned instruction followed and teacher influenced.

Missing Evidence: • Lesson plan posted and is related to what is observed. (School-Age classroom does not have a Lesson Plan posted. Lesson Plan in Threes Classroom only partially completed.)

E1.8: Review of Curriculum/Lesson Planning

Missing Evidence: When the Directors were asked about their review of the educators lesson planning, the directors do not have documentation that they have reviewed the lesson plan their educators are teaching. The Three Classroom Lesson Plan that was posted was only partially completed. The directors have not reviewed this lesson plan or what the educator will be teaching.

E3.3: Clear & Appropriate behavior expectations are given.

Missing Evidence: • Classroom rules are posted and reviewed with the children to teach and reinforce appropriate behavior. (Playground Rules posted in School-Age Classroom, but not classroom rules. One playground rule states "children must stay on playground unless accompanied by a teacher.")
