Summary of Complaint Visit Complaint Visit #PCS1002696 Technical Assistance Provided: 1. Director and LC discussed protocol associated with Supervision Letters provided by Background 2. LC suggested using small baskets at the tables, during mealtimes for younger children. The children would use these to throw snack and meal items away, in order to provide them with a sense of responsibility and would also help the educator with clean-up duties at the same time. No Violations were received

Observation: Bus used to transport children on field trips had a fire extinguisher that had not been serviced in several years and had expired. 1240-04-01-.17 Transportation Corrective Action: Agency agreed to immediately buy a new fire extinguisher and place on vehicle prior to it being used to transport. Technical assistance given.

On the date of this visit the driver qualifications were checked and the driver did not have a drug screen 10 days prior to assuming duties as required by the rules. 1240-04-01-.17 Transportation Corrective Action: Driver was removed from duties and will not transport until a drug screen is completed and the results are on file. Driver will assume duties with a negative drug screen. Technical assistance given.
