See Below for Detailed Information Observation: On March 24, 2002 while conducting an unannounced self-reported complaint investigation, LC April Cline learned that a four year old child was left alone in the classroom while the rest of the class went to the playground. The child was in the cozy area as she was not quite ready to be up from nap. Educator Haji was cleaning tables from snack while educator Wilson was having the children put on their jackets and line up. Upon exiting the classroom, neither educator performed a head count or name to face identification. Once the children and the educators were at the playground gate, a head count was done by educator Wilson and she realized there was one child missing. Educator Haji returned to the building to get the child. Another educator had found the child to be alone and was with her. (3) Playground Supervision (b) Child care agencies shall develop and follow a written playground supervision plan that includes: 1. Arrival and departure procedures; 6. Roll call before leaving classroom and upon arrival at playground and prior to leaving playground and upon arrival in classroom. Corrective Action: the 2 employees were placed on a safety plan, training was received by CCR&R as well as by the Director. A Plan of Corrective Action was developed as well as a Put on Notice Letter and these were displayed in the agency and parent signatures were gathered. The 2 educators also received formal write ups to be placed in their employment file. (a) To ensure the health and safety of all children enrolled, the management of the child care agency shall maintain a system that enables all children to receive a level of supervision that is appropriate to their age and their developmental, physical and mental status. The child care agency staff shall know where children in their care are at all times, be aware of their activities, and be able to intervene appropriately. (c) If any child is left unattended at any time, the child care agency is subject to summary suspension, denial or revocation of the license. This includes, but is not limited to a child: 2. Being left in a classroom or any other area of the facility; Corrective Action: the 2 employees were placed on a safety plan, training was received by CCR&R as well as by the Director. A Plan of Corrective Action was developed as well as a Put on Notice Letter and these were displayed in the agency and parent signatures were gathered. Observation: The educator disciplined a two year old child by placing him in time-out or "time-away" and forgot about him resultiing in him being left in the classroom alone. 1230-04-01-.15 PROGRAM, LANGUAGE, AND LITERACY DEVELOPMENT. Corrective Action: The agency director is aware that no children under the age of 36 months shall be placed in time-out. Management will conduct a staff meeting on 1/28/19 to train staff regarding the acceptable use of time-out. Technical Assistance given.

A parent found him and reported it to staff nearby. 1230-04-01-.11 SUPERVISION Corrective Action- The agency is implementing a policy that face to name check will be conducted every 15 minutes in all classrooms. On 1/28/19 management will review the violations and new policies with staff. On 2/5/19 DHS will conduct training with all staff. Technical assistance given.
